A refinement is a fresh short series of aligners made from a new scan near the end of treatment, targeting whatever didn't fully express from the original plan. The digital plan predicts tooth movement; biology executes it — and teeth are individuals. A stubborn rotation, a tooth that moved slower than modelled, tracking lost during a low-compliance month: any of these leaves a gap between plan and mouth. Industry-wide, a substantial minority of cases — commonly cited around 30–50% — use at least one refinement round. It's a designed-in phase, not a malpractice signal.
The honest logistics: a refinement needs a new scan. For our patients that means either a short return visit — many fold it into a holiday or family trip anyway (NRIs especially) — or an iTero scan at a practice near home forwarded to your treating orthodontist, an increasingly available option in major Western cities. New trays are manufactured by Align and shipped to you; monitoring resumes remotely. Depends entirely on the quote you signed. Comprehensive plans (including our partner clinic's) include clinically planned refinements; budget and per-aligner pricing frequently bills them separately — a hidden-cost hotspot worth asking about anywhere you get quoted.
One round settles most cases; complex bites occasionally use two. If a case keeps needing rounds, the honest conversation is about compliance or about whether the movement goal needs rethinking — not about ordering more plastic.
